Add a rating and a Likert matrix to a form

Feedback, satisfaction, and training forms usually come down to two questions: how would you rate this, and how much do you agree with these statements. A rating field and a Likert matrix cover both, and the answers save to your SharePoint list like any other field.

The rating field

A rating captures a single score, usually a row of stars. It suits one clear question: rate the session, the service, or the venue. It stores a number, so you can average it and chart it later without any cleanup. You will find a rating field in the builder ready to drop onto a form.

The Likert matrix

When you want several statements answered on the same agree-to-disagree scale, a Likert matrix keeps them in one tidy grid instead of a long stack of separate questions. The Likert matrix is a Forms365 Workspace control. It looks like this:

Each row is stored as its own answer, so a report can look at a single statement across every response, not just an overall blob.

Putting a short survey together

    Add the questions

    Drop a rating for the headline score, and a Likert matrix for the statements you want measured on one scale.

    Set the scale wording

    Use the labels your team recognises, whether that is agree-to-disagree or a one-to-five scale.

    Save to the list

    Connect the form to a SharePoint list so each response lands as an item, ready to filter and chart.

    Preview and publish

    Fill it in yourself once to check the scale reads well on a phone, then publish it to the people you are surveying.
